WHAT WE DO

Three needs that hold people back —
handled together.

Housing, transportation, and health are tangled together. Lose one and the others slip too. Meridian works on all three at once so progress in one area doesn't unravel the rest.

Stable housing

For people facing housing instability or homelessness for the first time, we help find and keep a place to live.

  • Housing-stability navigation
  • Benefits screening & enrollment
  • Connection to local resources

Rides to care

A missed appointment can undo months of progress. We arrange the transportation that gets people there.

  • Transportation to medical visits
  • Rides to benefits & housing appointments
  • Help navigating local transit options

Healthy food & habits

Good health is built daily. We connect people to nutritious food and support lasting lifestyle changes.

  • Food-access screening & referrals
  • Pathways to medically tailored meals
  • Coaching on healthy daily routines
WHO WE SERVE

Open to anyone in need.
Built to honor those who served.

Meridian serves all Baltimore City residents facing housing, transportation, or health barriers — no one is turned away for who they are or where they've been.

We pay special attention to veterans, whose service earns a clear path to the housing and health support they deserve.

OUR APPROACH

Screen. Connect. Follow up.

A simple model that doesn't stop at a referral. We stay involved until the connection actually holds — because handing someone a phone number isn't the same as solving the problem.

01

We screen

A short, respectful conversation surfaces what's really standing in the way — housing, a ride, food, or all three.

02

We connect

We match each need to the right service and make a warm handoff to partners across Baltimore, not a cold list.

03

We follow up

We check back over the following weeks to confirm it worked, adjust the plan, and keep people moving forward.

Everyone deserves a stable place to begin.

Whether you need support yourself, want to refer a neighbor, or hope to help fund the work — there's a place for you at Meridian.
